Scenario 2 – Family 2 Part A – Family and Child Support Plan India (3 years) has just started at your centre and attends the service three days a week. India has down syndrome with global delay and comes from a two parent family and both work fulltime. For the past two months Eyla (India’s mother) has had her brother living with them after returning from work overseas and he cares for her on the other three days to help the family. India often attends the service with her hair mattered with knots. Today she arrived with numerous small bruises on her upper leg and forearms. India smelt like she hadn’t had a bath or shower and a red rash was present around her mouth. Whilst you were supervising the bathroom you noticed that when India urinated, she said ‘Ahhh” and then started to cry saying “it hurts when I wee”. You said “can you show me where it hurts” and she pointed to her vagina. You asked “when did it start to hurt?”, she shrugged. “Did it hurt this morning”, and she nodded. You helped her dry herself with toilet paper, pull up her pants and wash her hands. When mum came to pick India up the director spoke to the mother in a caring manner and suggested India see a GP as it was hurting when she was passing urine. You also said that India was saying the rash around her mouth was also very sore and red. You had applied cream today to help with this. Mum said thank you for doing that and she would take her to the doctor now. Two days later when India returned to the centre, she had not attended the doctor, she cried when passing urine and you noticed blood on the toilet paper. She cried and only wanted to cuddle one educator today. As the educator helped India onto her lap for a cuddle India said “Ahh stop it, it hurts” The educator asked, “What hurts India?” “My arm” and India pointed to her right shoulder. The educator asked if she could have a look and India agreed. The educator went to remove her shirt however noticed when she lifted India’s right arm she screamed in pain. The Educator inspected her shoulder from above and found that it appeared to be dislocated. She asked India if she was able to lift her own arm however was unable to lift her arm independently when instructed by the educator. The educator asks India “How did you hurt your arm?” She just looks down and says I don’t know” You have decided that today you need to get immediate medical assistance for this child. One of the educators in her room has stated to you that “these parents should lose their child right now”. You will also need to address this issue. (See Part B below)
