From my understanding, this patient has previously taken a prophylactic antibiotic prior to past dental procedures; is currently taking a blood thinner, discontinuing use a few days prior to treatments; is prescribed a diuretic and carries nitroglycerin, in the form of a spray to use in the event of angina. Antibiotic prophylaxis (AP) is “prescribing an antibiotic to a patient to prevent bacterial colonization while undergoing dental treatment. For many years, patients with certain medical conditions (specifically, cardiac conditions and prosthetic joint implants) were prescribed an antibiotic to be taken prior to dental treatment (Robinson, 2023, p. 410).” Common antibiotics used for prophylactic treatment are amoxicillin, ampicillin, cefazolin, or ceftriaxone. Blood thinners or anticoagulants are used to prevent strokes caused by blood clots. Blood thinners are likely to interfere with dental procedures, meaning the staff must know if the patient is on this type of medication (Robinson, 2023, p. 411). Diuretics are a classification of cardiovascular medication that are responsible for reducing fluid retention and decreasing blood pressure. Finally, a nitroglycerin spray is a vasodilator and may be carried by an individual that experiences chest pain (angina). I would express to the dental assisting intern that it is vital to get an updated health history from every patient. But an accurate health history is even more important in an individual that has previously taken medications notorious for interfering with dental procedures and materials.
