Original Question
M3 discussion reply2 N PINO Below are three patients that are admitted to your unit. For each patient, you must identify who on your team should be assigned to each patient and provide a rationale for why. Client 1: Room 422 A: A 49-year-old male client admitted two days ago with hypertension and peripheral vascular disease. The client underwent a stenting procedure to the left popliteal artery one day ago. The client is expected to be discharged home later today. For this patient I would assign the LPN. This patient is being discharged and should have their discharge instructions reinforced by nursing staff. While discharge teaching is the responsibility of the RN, it is within the scope of the LPN to reinforce that teaching prior to discharge (American Nurses Association [ANA], 2019). Neither of these tasks is appropriate for assistive personnel. Client 2: Room 423: A 70-year-old female admitted yesterday with a diagnosis of community acquired pneumonia. She is afebrile, with stable vital signs. She has a saline lock in place and is receiving IV antibiotics every 8 hours. She is on oxygen at 2L/minute; her oxygen saturations have been 93-95% on the previous shift. Assigning the AP to this patient would be suitable for this patient. She is afebrile with stable vital signs and is unlikely to need further assessment or intervention outside of scheduled antibiotics. The AP can assist with ADLs and the general needs of this patient. This does not delegate any part of the nursing process, nor does it require the AP to make any clinical judgements (American Nurses Association [ANA], 2013). Client 3: Room 424: A 67-year-old female admitted with an exacerbation of COPD three days ago. The client is receiving IV solumedrol every 8 hours and is on oxygen at 2L/minute nasal cannula with oxygen saturations of 89%. This patient should be assigned to the RN. Of the three, this patient is most likely to deteriorate and need further assessment and intervention. While a SpO2 of 89% may be near the patient’s baseline due to her diagnosis of COPD, she may need her oxygen titrated based on clinical presentation or other treatment such as a bronchodilator. Assessment and intervention in this patient are not appropriate to delegate to the LPN or UAP. QUESTION: Response to Classmates You are required to make responses.
