Case Study 1: Maggie is a 42-year-old female who is a highly successful manager of a large company. She works long hours and often skips meals and states she lives on black coffee and nicotine. She has frequent headaches that she puts down to stress, for which she takes Ibuprofen. Maggie has been feeling increasingly tired and even a little dizzy at times. Her bowel actions have been black and tarry, however she thought this was a result of her poor diet. Maggie was brought into hospital after an episode of suddenly feeling weak, lightheaded and nauseated. She began vomiting what appeared to be large amounts of bright red blood and was found unconscious. Assessment – Alert and orientated, very anxious and complaints of 8/10 epigastric pain. Her skin is pale and cool. Vital signs on admission – BP 89/49, HR 120, RR26, O2 sats 98%, temp 36.0C. The patient has had another 2 episodes of haematemesis approx. 80 – 100 ml. IVT had been commenced – Hartmann’s 6/24 and the patient has been given Morphine and Maxolon. A blood test was taken, and the results show her haemoglobin (Hb) levels to be 100g/L 1.
