Activated T and B lymphocytes infiltrate the synovium and produce pro-inflammatory cytokines such as TNF-α, IL-6 and autoantibodies, including rheumatoid factor anti-CCP antibodies. This immune activation leads to pannus formation, an aggressive synovial tissue that eroded cartilage and bone, causing joint destruction, deformity, and systemic symptoms like fatigue and weight loss (Jin et al., 2022). Unlike OA, which progresses slowly and primarily affects load-bearing joints such as knees and hips, RA typically involves symmetric small joints, especially in the hands and wrists.
