45 year old new male patient presents to clinic for allergy/asthma concerns. He reports he has hx of severe allergies/asthma as a child when he lived in Texas, then moved to California and s/s resolved. He reports he recently moved back to Texas and has had recurrence of allergies and asthma. Reports daily nasal congestion, post-nasal drip, worsened at night while lying down, and dyspnea with audible wheezing and tightness in upper airways. Reports he has an albuterol inhaler which is the only thing that has relieved his dyspnea in the evenings. He is down to his last few puffs in his inhaler. Patient is requesting a refill of albuterol. He denies taking any other medications. He reports that his family tries to use all “holistic” medications. Patient required allergy injections in childhood for 4 years- patient reports that he does not feel that this was effective.
