Write H&P. Mom says the teacher recommended evaluation for Jaden because he’s struggling in class, not getting along with his peers and teacher believes he will need IEP to help him succeed in school. Mom says she noticed his behavior at home pretty early and teacher says he has a lot of meltdowns in the classroom, he’s rigid with his routine, not getting along with his peers. He’s in 1st grade. He doesn’t like his routine changed. Change triggers his behavior and arms flapping. At home it takes some time to calm him down, figure out what’s going on, sometimes redirecting him towards his Lego’s help. In school he’s distracting for other kids. Outbursts are severe but he’s li to his Lego’s and he’s trying to take them everywhere to school but we encourage him not to bring it to school and then he has a meltdown. No neuropsychological tests done, school asked to come here to do an evaluation no genetic test done, pediatrician never recommended any tests. Daily routine he gets up around same time every day, eat breakfast, get ready for school, goes to school, comes home, snack, dinner we try to keep really familiar mom says. He has hard time understanding social cues, understanding communication and interactions and has problems with back and forth conversations. He doesn’t socialize with people especially those he doesn’t know. Legos are his favorite thing to do. He has no friends he ignores questions just play with his legos. Teacher says he doesn’t get along with his classmates. He performs at school very well, he started reading when he was 2, no academic concerns just behavioral. Jaden when asked about his school and friends no answer just moves his shoulders he says he likes Legos and he likes to line them up by colors. His favorite color is blue. He says he gets upset in school because it’s a change and he feel safe “just when things don’t change” He says he feels “just normal”. Parents married 20yrs. Mom works in finance dad is a surgeon. Live in single family home. Jaden doesn’t care to interact with other family members. Parents are his support and teachers. Deist- good variety of foods eats x3/day plus snacks twice a day. Only exercise is his PE at school. Goes to sleep around 8:30 and gets up at 0700. Sleeps through the night. Met his developmental milestone except delayed walking and potty training. Mom no preexisting medical condition no medical problems in pregnancy no medication except prenatal supplements no external stressors. She walked 20-30 min everyday during pregnancy as an exercise. No problems on USG Regular doctor visits during pregnancy. He delivered him at 42years old. No complications during delivery. His apgar score was normal. No medical interventions.
