Write an Initial Psychiatric Evaluation/ diagnosis, Psychiatric visit HPI, social, screening tools, MSE, Family History, current psych meds, diagnosis and recommended Plan for a 89years old female who is currently living at Sharon care skilled facility. She has physical diagnosis of Acid reflux, anemia, cataract, ENCOUNTER FOR SURGICAL AFTERCARE FOLLOWING SURGERY ON THE DIGESTIVE SYSTEM. She is also receiving PT for strengthening. She was admitted to Sharon care 2/18/2025. She has past psychiatric history of MDD and she is on Trazodone 100mg at HS. Patient said she lives by herself at home but my daughter visits ofen. Patient said I was very depressed when I had my surgery in the hospital but I am now hopeful that I will be going home soon. Patient said I take Trazodone for sleep and it helps me to sleep well. She said however, she needs to stop watching TV at bed tine to improve her sleep. Patient said her appetite has improved but it was worst after my surgery. This morning I ate all my breakfast, I am getting better everyday. She said she also had Covid and has completed treatment and hopes all the symptoms go away soon. She said she has 3 children, 5grand children and 1 great grand child, so I have a lot to live for. Orders are to decrease Trazodone to 50mg due to improved mood and improved sleep pattern. Education on good sleep hygiene. Her PHQ was 10 indicating moderate depression.
