Write history of illness for a 79years old female who lives in Cottesmore of life care. Pt was admitted in the ER two weeks ago for visual hallucaination. Pt states that this started on Friday night the 17th of January. She said she saw a man looking at her while she was doing her income tax papers. She said she ;later saw another man looking at her with groceries in his hand. She said she continued seeing witchy looking people in her house. She said on January 25th, she saw a woman with a caqt on her neck and she was so was scared and called 911 and they took her to the emergency room. She stated that she has been at life care of Cottesmore since January 25th. Patient has past history of depression. She has weakness of lower legs, receiving therapy. . Pt said she used to love to garden, but she cannot anymore because of her legs and that makes her sad.
