He is mentally alert, although remains angry that he has been unable to return to his home. He prefers staff to address him as Mr. Simcoe. Hugh is not a candidate for a prosthesis and so can only be mobilized to a wheelchair. Currently he is on complete bed rest as he has a Loonie-sized secondary pressure ulcer on his coccyx, sustained on his final readmission to acute care. His family is paying for a Kinair Bed and he has a Foley catheter in situ to keep his skin dry; however, recurrent urinary tract infections (UTIs) are a concern. His current vitals signs are: T-37.6, P-84, R-18 BP 130/84 -Mr. S. sleeps very poorly at night. He is unsettled in his new surroundings and his pain is relentless. She is mentally alert but has decreased vision secondary to cataracts and decreased hearing. She is not a candidate for cataract surgery and her vision has decreased to the point where she has difficulty seeing food on her plate. Her hearing aids were lost and not replaced 6 months ago when she went to the hospital due to aspiration pneumonia. Secondary to cerebral vascular accident she experienced 2 years ago, she has limited manual dexterity, needs assistance feeding herself, and is on a pureed diet due to dysphagia. She has lost 17 pounds over the past 3 months, leaving her 5 foot, 4-inch frame at 90 pounds. Her dentures are ill fitting and too painful to wear. She states “I don’t enjoy food anymore”. .
