Treatment plan with goals and objective Amanda is a 35-year-old female who is familiar to VBHCS and was under our care in 2012-2014. She is in the process of getting her children back from DCS, who are 11, 9 and 4 years old (the 4 year old child is currently separated from her two siblings). Client reports ongoing mental health problems throughout her life. Her father has alcohol use disorder, severe and was physically and mentally abusive to client throughout her childhood, and continues to be verbally abusive. She denies ongoing nightmares, but reports she does have occasional vivid dreams of her past abuse . Client lives with her mother is very supportive of client and her children. Amanda reports ongoing depression, 7/10 with feelings of worthlessness, hopelessness and “wanting to get her children back.” She reports fatigue, difficulty with concentration, lack of pleasure/interest, and poor sleep and appetite. Client is requesting counseling services only at this point in her care, and states she “does not like to take pills and would prefer not too.” She denies SI, HI or acts of self-harm. Client reports self-harm behavior in her past (age 14), cutting, but has not self-harmed for many years.
