This is a 76-year-old male with history of ischemic cardiomyopathy, status post bypass surgery and AODM. The patient has mild ankle edema which results after lying supine. The patient has no orthopnea or paroxysmal nocturnal dyspnea. He has no chest pain or evidence of ischemic heart disease. He also has no evidence of palpitations, dizziness, or history of syncope. His blood sugar has increased slightly since he was switched to generic diabetes medications. He feels fine otherwise. PMH: Significant for bypass in 2015, cholecystectomy and cataract surgery. Medications: K-Dur 20 mg BID, Micronase 45 mg QD, Lanoxin 0.125 mg QD, Demadex 100 mg QD, Accupril 20 mg QD, and multivitamins. Allergies: Sulfa drugs Social history: Non-smoker, drink 3-4 cups coffee/day. No alcohol Family history: Positive for ischemic heart disease Review of systems: Negative for TIAs, claudication and stroke. Positive for DM. Examination: Well developed, well nourished male, np apparent distress, alert and oriented. B/P 140/70 P 76 R 14 Head: Normocephalic and atraumatic Neck: Supple. No carotid bruits.
