You are the Support Worker working at Golden Hills Long Term Care Home. Thomas is a 91-year old gentleman who has lived at the residence for 3 years. He was admitted to the home directly from the hospital after a stroke. Over the last year, Thomas has become weaker and increasingly frail. He requires assistance with bathing, dressing, grooming, and dining. He values his independence and refuses to use a wheelchair when he is fatigued. He uses a rollator walker in the home area, and out in the corridors. The nurse charted last week that Thomas is showing early signs of Mixed Dementia (Alzheimer’s and stroke changes). As you prepare to enter the room, you hear some muffled moaning sounds from inside. You enter the room and find Thomas lying on the floor, face down. He is moaning and moving his arms slightly. You scan the room quickly and see that his left foot is caught in the fitted sheet. Thomas has a small 2cm by 2cm pool of blood under his forehead. The charge nurse Charlene arrives and begins to assess Thomas. “You sure bumped your head good Thomas”, Charlene says. Thomas smiles but insists he feels fine.
