t is the second weekend in March, and you are working the weekend drop-in clinic at the local urgent care with the usual “skeleton” staff. A fellow NP asks for your assistance and insight into a sick visit that they are seeing. Per the other NP, the sick visit is a “triple”, in which three children from the same family are being seen for an unusual set of symptoms. Per the children’s parents, “Remy,” their 4-year-old daughter, is presenting with a day history of fever, cough, cold symptoms, with runny eyes. Their second child, “Jeremiah,” a 2-year-old boy with a four-day history of persistent fever, mouth sores, and is now developing a rash. And their third child, a 9-month-old boy named “Zeke”, just started this morning with a runny nose and fussiness. Per the parents, Remy seems to follow the same path as Jeremiah. Jeremiah started four days ago with fever (up to 102F) which has increased to 105F with the onset of the rash this morning. His rash started behind his ears and along his hairline and has since spread to his face and neck. Jeremiah’s appetite has decreased significantly with the onset of the spots in his mouth.
