response to my peer what do you think ? During my clinical experience, I often see nurses refer to frequent patients as “regulars” on the unit, especially at the Allen. These patients usually have multiple chronic illnesses that all together, require a very specific management plan that can consume lot of the restricted time and energy these patients have because of their illness. Unfortunately, I have not witnessed any nurses utilize this top-down, shared decision making approach when informing a patient of their discharge treatment plan, however I do believe basically all of the patients I have encountered would benefit from this method. The portion of the article where Susan sits with Harvey to discuss what exactly may have lead to his fluid overload really reminded me of my psych rotation. Different forms of therapeutic communication were often used in Psych, which allowed the nurse and the patient to building an understanding of what exactly is affecting their current state. Susan starts her conversation with Harvey with an open ended question, which not only allows Harvey to feel included in on the plan at hand, but also allows for a dialogue that surfaces the specific obstacles he’s facing. I think this approach should be used more often as the nurse can learn to cater to the patient as a individual, rather than a set disease.
