When the patient initially noticed the dyspnea, it was only with activities such as climbing the stairs in his home. He believed that this was related to a recent upper respiratory tract infection and thought that it would get better with time. However, the dyspnea did not improve and now the patient endorses difficulties with performing daily activities such as walking from bed to the bathroom in the morning or getting dressed related to the shortness of breath. When asked if he has experienced any other symptoms, the patient endorses that he feels more fatigued early in the day despite getting an adequate amount of sleep each night and most recently has noticed peripheral edema in his feet and ankles. The patient was concerned because they had a family history of pulmonary hypertension and these symptoms seemed to correlate.
