Preface: I am trying to propose an EBP Change Process using the ACE model of Knowledge Transformation. The topic that I am using is CHF hospital readmissions and methods of nursing practice discharge education interventions. I would like to initiate a practice change to include teach-back method in the discharge education provided to the patient/caregiver to increase compliance and self-management of CHF by the patient, which I believe would reduce readmission rates. I am stuck on how to answer these questions. I have found 10+ articles but, I do not know how to interpret and apply them to the questions asked. Thank you! I appreciate your assistance! This is my PICOT question, but I think that it may need tweaked. Among patients age 65+ discharging from acute hospital setting for CHF exacerbation(P), does discharge teaching including the teach back method(I), compared to discharge teaching not including the teach back method (C), reduce the number of 30-day heart failure readmissions (O) within 6 months?
