The Agency for Healthcare Research and Quality (AHRQ) encourages collaboration across different disciplines to seek solutions that provide a culture of safety for patients. Patient-centered care ensures that transitions between providers, departments, and health care settings are respectful, coordinated, and efficient. Information is critical to patient-centered care and is a field of health informatics that focuses on how information is acquired, stored, and used in health care, with a particular emphasis on technology. Clinicians can use informatics to integrate the information they learn from patients with their medical knowledge and data resources to improve patient care (Hannawa et al., 2022). Patient safety is essential to high-quality healthcare and should be the focus of any technology introduced in a patient care setting. Continuity of patient care is attained by the concise and clear transfer of patient’s clinical information from one healthcare provider to another (Ghosh et al., 2021). Communication failure can significantly lead to serious medical errors. The recent patient situation that I chose to reflect on involves a patient who was suffering from cardiac arrhythmias. The patient’s health was significantly deteriorating and he required urgent medical attention since his required consistent monitoring. Three communication tools that were used to provide quality care in the patient situation were closed loop communication, Introduction, Situation, Background, Assessment, Recommendation, Read-back (ISBARR), and Handoff Reports. These different communication tools helped to facilitate accurate and clear information exchange. Furthermore, they helped to facilitate effective decision-making. ISBARR was important in ensuring that all necessary information was conveyed during critical situations and handoffs. The communication tool helped the healthcare providers to discuss the condition of the patient and the required actions. Handoff reports facilitated the continuity of care since they helped to communicate important patient information during shift changes. This was important in ensuring that the interprofessional team was up-to-date on the health status of the patient and his treatment plans. Lastly, the closed-loop communication was important in ensuring that the recipient and sender were on the same page. Furthermore, it helped to reduce cases associated with miscommunication as every member of the interprofessional team understood his or her roles correctly. An example of how technology in the provided patient situation could be improved is by promoting interoperability of electronic health records (EHR). This will help to promote data sharing and more seamless integration between the utilized EHR systems (Ghosh et al., 2021).
