Original Question

The patient was brought in the operating room and placed on the operating room table in a supine position. General anesthesia was administered endotracheally with a special laser tube. The left anterior tongue, floor of mouth, and alveolar area of tumor were seen and examined circumferentially. With margins this was marked with Bovie cautery prior to injecting with 1% lidocaine with 1:100,000 epinephrine. Using a hand-held laser set at 15 watts, super pulse power was utilized around the circumference of the lesion, and dysplasia was circumscribed with the laser as well. The central tumor and dysplastic tissue were then ablated and vaporized with the laser. Specimens were sent from the left anterior tongue, anterior and posterior alveolar mucosal margins, and left floor of mouth deep margins from the patient after ablation of the tumor. All these returned back negative for residual squamous cell carcinoma except the posterior alveolar margin that showed one small foci of carcinoma.
