Original Question
a. As a nurse leader, what strategies can you implement to balance the cost of quality with client outcomes? There are many strategies that can be implemented as a nurse leader to balance the cost of care. In my previous role, I was a Surgical Services manager overseeing four departments. One of the main requests from C- Suite was cost reduction, improve patient satisfaction and maintain appropriate productivity. Managing staff productivity is tricky as you want to make sure that you’re staffed adequately throughout your day to maintain adequate staff to patient ratios. In surgery, if we were poorly staffed, we created a bottleneck effect which ultimately resulted in delayed surgical cases resulting in more expenses. In surgery, our biggest expense is surgical OR minutes and Anesthesia minutes. The last thing we need is having Anesthesia idling as they pull the biggest expense. One of the things that I did to balance out costs was to generate reports through EPIC to gauge out or busiest times of the days. I was able to trend accordingly and make staffing schedules to reflect our busiest times. According to an article written in HFMA, ” A high-level assessment that begins broadly, then quickly dives deep into areas of clear opportunity is often the most effective approach.” (HFMA, 2022) It is imperative to assess need and areas that are increasing overall costs in order to address long term needs. Other areas that were a drain to department costs were surgical supplies that were used but not charged to a surgical case. Over time that was millions of dollars of lost revenue. b. What metrics are used to measure the cost of quality? According to ThroughSpot, “Patient satisfaction is a critical indicator of the quality of healthcare services provided. It reflects how well healthcare providers meet patients’ expectations and needs, including aspects such as communication, wait times, and overall experience. A high patient satisfaction score indicates that patients are receiving satisfactory care and are more likely to have positive outcomes.”(TS, 2025) The healthcare system in the United States utilize Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) to survey patients after their discharge. Surveys are sent to a random group of patients, and it covers essential topics such as pain control and accessibility to their healthcare provider. When surveys are completed, they are analyzed, and a score is sent to the healthcare organization. Healthcare organizations that receive higher scores are incentivized and given full insurance reimbursement. c. The cost of quality can be a significant barrier to improving client outcomes in resource-limited settings. What innovative cost-saving strategies can be implemented to enhance quality care delivery in such settings? One of the ways to improve client outcomes in resource limited settings is to audit your department for cost savings. One of the principal ways to reduce costs is to prevent unnecessary waste. In surgery, we realized that our wastes were derived from opening supplies that were not utilized in cases that were then tossed away. The supplies were then not charged because they were not used. Surgical preference cards were updated to show that under-utilized supplies were to be held and not open unless requested by surgeon. All staff were re-trained in the handling of supplies and were educated on charges. The effects were felt almost immediately as there was a decrease in supply orders as they weren’t wasted.
