Need reply for this discussion post- A situation in clinical which I was concerned was when I had multiple patients who were high fall risks and they were very prone to wanting to get out of bed, was on sedatives, and were highly agitated. My nurse was increasingly getting concerned over the situation because the more the patients were agitated the more they were likely to have a higher risk of a fall. I noticed that my nurse was then having the certified nursing assistant check on these patients more frequently and my nurse and I were also checking on them frequently as well as checking the bed alarms, slip-free socks, and bracelets were on signifying fall risk and that their call light was in reach. Falls are such a serious matter because they are one of the leading causes of injury in hospitals and helping to prevent them includes identifying the risk and using evidence-based decision making (Houser, 2017). My nurse then made a decision that we were going to get different bed alarms so that we had different alarms sounding as well as the alarm built into the bed that way we could be alerted in different ways. My nurse also decided to educate the patients on the risks that were involved with them repeatedly trying to get up out of bed and not contacting us to assist them when ambulating. My nurse utilized the nursing process with evaluating the situation and then gathered evidence from the charts, her own personal experiences, the charge nurse and she even contacted the physical therapist in order to find better ways to benefit the patients. The nursing process helps the nurse because they are required to gather evidence and critically think about how to apply this to the patient’s situation in order to have positive outcomes (Slade et al., 2017). We were able to improve the situation by reducing the fall risk in ways such as promoting communication between the patient and the care team as well as ensuring that they understood the risks and consequences of falling.
