NEED A CARE PLAN FOR MR. P AND PLEASE PROVIDE IN TEXT CITATIONS AND REFERENCES in apa. Mr. P is 69 a 69-year-old male, born on 07/12/1955 in Toronto, Ontario and moved to Calgary in 1967 and has lived in Calgary since then. He worked in building oil rigs in various parts of Alberta. Currently, he lives alone in a private residence, not taking any home oxygen therapy. He was married twice, got divorced from his first wife and is now widowed from his second marriage. He is estranged from his one child, not sure whether female or male, who isn’t in contact. he doesn’t have many friends either, just a couple. He likes watching ice hockey, and he is a fan of the Edmonton Oilers. He used to do fishing, camping and hunting when he was young. He has been diagnosed with COPD, prostate cancer, hepatitis C, chronic Hypertension, declining functional status, Alcohol use disorder, chronic hyponatremia, hypoxia, osteoporosis, and COPD exacerbation. He has received the HAV vaccine (Hepatitis A) on 2/12/2011 and 31/03/2011. He is allergic to orange and cedarwood during certain seasons. He is 131 kgs, 41.44 BMI kg/m2, 177.8 cm Ht. He smokes every day and goes through 50 packs a year. He consumes 686 ml of alcohol a week, or eight beers a day. He ended up in the emergency department under the influence of alcohol and suffered an unclear injury resulting in a right, closed subtrochanteric femur fracture due to pain and inability to ambulate. EMS was called, and they transferred him to the hospital’s emergency department. After evaluation, he was diagnosed with a right hip fracture. He described the injury as a ground-level fall, could not tell when and how it happened and was unable to get back up, had a very painful right hip and right lower limb. He denied any loss of consciousness and any pain to the head and neck. He has been admitted to the orthopedic unit for pain management after surgery. Medically stable, as he had the right femur open reduction internal fixation surgery due to a subtrochanteric fracture of the femur, but pulmonary has been diagnosed with new interstitial lung disease.
