Value based care and payment systems will be increased at a federal level to increase adoption, allowing for a focus of quality and better patient outcomes. The initiation of the Patient Protection and Affordable Care Act (PPACA or ACA) had a profound influence on state and local healthcare reform efforts in several ways including expansion of Medicaid, health insurance exchanges, and innovation. The ACA originally mandated states to expand Medicaid services within the federal poverty level, with the federal government covering much of the cost (Sommer, 2019). The Supreme Cort overruled this mandate, making it optional for states, leading to an increase in access to healthcare for the low-income population (Sommer, 2019). ACA mandate of using marketplaces where individuals can purchase health insurance, allowing for the use of state or federal exchange options with an aim to increase competition, increase transparency and allow for policies to be more affordable (Sommer, 2019). The program sets an annual goal for healthcare spending within the state, helping to control the rise in costs and encourage facilities and providers to operate more cost efficiently (Rakotoniaina, 2022). The program also, measures the improvement of quality of care provided through metric reporting to ensure cost savings does not sacrifice quality (Rakotoniaina, 2022).
