Introduction Hip fracture is a distressing and life-changing event for patients. Treatment, care and rehabilitation is governed by evidence-based recommendations, and patients’ preferences are sparsely represented. The aim of this systematic review was to identify what elderly hip fracture patients consider important in relation to their fracture. Methods included studies presenting interviews of patients with hip fracture aged >65 years describing patients’ perspectives on treatment, care and rehabilitation. A qualitative approach was applied to present findings. Data were extracted by both authors and categorized as either health-related factors or healthcare-related experiences. All studies were assessed using the CASP checklist. The protocol was registered with PROSPERO (ID CRD42018091981) Results Seventeen qualitative studies were included. Health-related factors included: symptoms and complications, physical health, mental health and social relationships, and personal goals. Healthcare-related experiences revolved around: waiting time, information, participation and respect, and discharge. Conclusions Hip fracture patients give priority to regaining physical functioning, mobility and independence. Their social network, a surplus of mental resources and the reduction of pain and complications are vital.
