Shirley is referred to you by her primary care physician, who expressed concerns following her multiple requests for pain medication refills in a short time span. Shirley has a medical history to include thyroid disorder and history of migraines. In the past she has been on pain medications for extended periods due to hip pain that was not resolved by surgery. Shirley states that she is “always in pain” and often requests extended pain prescriptions from her doctor. When asked about her prescription medication use, she does admit to taking more Percocet than prescribed. Shirley is recently widowed, having lost her husband of 45 years one year ago to a heart attack. She has 3 children and 6 grandchildren, but no family close by. Shirley appears to be dismissive of questions related to her use of Percocet and rationalizes her use due to pain. She does report that she no longer attends her gardening club and dinner club. When questioned further, she stated that her medication may contribute to her lack of interest and motivation to socialize. She reported a decline in church attendance as well.
