INDICATIONS: Dwight is a 58-year-old man who has a left epidural hematoma and presents today for surgical intervention. We have discussed any and all risk factors and he understands and agrees to proceed at this time. POSTOPERATIVE DIAGNOSIS: Same. PROCEDURE PERFORMED: Craniectomy, removal of epidural hematoma ANESTHESIA: General PROCEDURE: Under general anesthesia the patient’s head was prepped and draped in the usual manner. The patient was placed in the head outrigger and we then prepped and shaved his head. An inverted U-shaped incision was done over the left posterior parietal area. The skin was incised. We saw the skin and hematoma above when we encountered the fracture. There was a free fragment of the fracture, I elevated this and there was a large epidural hematoma there. We evacuated some, stopped the bleeder on the meninges, and then proceeded to irrigate the wound well. I then beeswaxed the bony edges, achieved hemostasis, tacked up the dura with 2-0 Vicryl and was satisfied that we would now close the scalp. I placed a Hemovac drain in the wound, closed the scalp and galea in layers with 0-Vicryl and surgical staples on the skin. A dressing was applied. The patient was discharged to the recovery room. Pathology Report Later Indicated: Hematoma.
