Ti… Show more Patient Situation: A 67 year old male with a history of chronic heart failure was brought to the Emergency Department by a neighbor. He has complaints of shortness of breath, especially at night and when walking, fatigue and swelling of his ankles and feet. He is in moderate respiratory distress. He is diagnosed with an exacerbation of his chronic heart failure and admitted to a Telemetry Unit for treatment. Patient Background: The patient states that he has not taken his water pill for the last 5 days because his wife’s ankles were swollen so he gave her the pills. He also states that he is out of one of his heart pills but cannot remember which one. He verbalizes that he is on several heart medications. He did not bring any of his medications to the ED. His physical examination revealed mild respiratory distress and edema of the ankles and feet. The 12-lead electrocardiogram showed sinus tachycardia with unifocal Premature Ventricular Complexes. His chest x-ray showed cardiomegaly. Arterial Blood Gases have been drawn. Results are pending. He is awake, alert, oriented and anxious. He is allergic to penicillin, cephalosporins and midazolam. He is a full code. He is a veteran. He cares for his 70 year old wife at home who is in the first stages of Alzheimer’s. Patient Assessment: The patient is exhibiting a heart rate of 100, B/P 158/100, RR 32; SpO2 is 84% on room air, temperature of 36.8 C. He appears cachectic and older than his stated age. He is awake, alert and oriented. His pupils are equal, round and reactive to light and accommodation. His cardiac rhythm is Sinus Tachycardia with unifocal PVC’s. He has circumoro and peripheral cyanosis. Skin is pale, cool and with poor turgor. Crackles can be heard in the bilateral lower lung fields. Movement is weak in all four extremities.
