write a SOAP note on the follow: “Client is a 28-year female who presents for psychotherapy follow up. Client is currently on a Medboard or Medical Evaluation Board (MEB) review status. Client has been serving in the military for seven and half years. Client states in the military, mental health is not discussed much due to stigma. Client is currently having a chronic back pain which is affecting her mental status. Client is currently on psychiatric medications (Prozac and Seroquel) and is also taking muscle relaxant and some other pain medication. These medications seem not to be effective as client is in constant pain. Client has experienced difficulties in her past relationships with friends and families. These struggles include misunderstanding, lack of trust, and communication breakdowns. As a result of these experiences, the client continues to experience self-doubt which has manifested in various ways such as client does not like to ask for help nor like to receive help, she states it scares her to rely on someone for anything Client elaborated how her chronic pain is affecting herself ego.
