Original Question
What? – (Describing the Experience) · Was the situation where interprofessional communication took place including who was involved, the setting and the specific communication that occurred? · Was the context of the communication such as patient’s condition, urgency or cultural factors that may have influenced the communication? · Was your role in the interaction? · Were your feelings during and after the communication? An incident happened whilst on placement on 16th January 2025, I was asked by my Practice Assessor to go to another ward to photocopy some papers. Upon entering the ward, I was met by a member of staff in the communal area, and I told her my reason for being on the ward. We both proceeded to the nursing office. I noticed a gentleman in the nursing office, with a sign on the door saying, ‘do not use this room’. The gentleman told us that it was ok to come in, as he had just applied the first coat of painting and was waiting for it to dry. A short while later, I heard a female, in a harsh tone of voice saying, “excuse me you can’t use that room”, in the presence of everyone. I came out of the office, apologised and explained that we were let into the office by the gentleman who was in there earlier. I told the lady I would come back when appropriate to do my photocopying, but she shouted once more “you will have to go to a different ward”. I then left and went to another ward to do the photocopy. The way the lady spoke to me left me humiliated, belittled and angry. I believed I had followed all protocols at that time. 2. So What? (Analysing the Experience) · was the impact on patient care: Did it enhance or hinder the quality of care provided? · did you learn about effective interprofessional communication. · What did you observe about the communication styles of others, and how did it influence your understanding of or yourself, staff and the service user? · Was your emotional response to the situation? How did it influence your behaviour or decisions during the communication? Having returned to my ward after the incident on the other ward, I informed my Practice Assessor about the incident but felt not particularly supported. The apparent lack of support made me more upset, humiliated and angry. In my moment of exasperation, I remarked to my Practice Assessor the unfortunate comment ‘what did I do to this Indian woman?’. The context in which I made my remark, in the presence of other staff, was that this Indian lady had treated me very badly. I did not intend my comment to cause offence or any racial slur at that time. Unfortunately, the staff around felt my comment was racist. NMC CODE 2018 (Section 20.1-20.3) I was later told that the lady who spoke to me was the manager of the ward where the incident happened. As a manager, I felt she could have communicated better than the way she spoke to me when she found me in the office. I also felt she did not give me a chance to listen to me. The comment which I made to my Practice Assessor in the office, sadly, presented me in a negative manner of other people’s perception of myself. I should have been aware of my emotional temperance and professional responsibilities. I accept that my management of anger and my perceived humiliation at that time was inadequate. I should have dealt with the incident on the other ward as one of a personality issue, rather than making a comment that was perceived by others as racial. Because of the comment that I made to my Practice Assessor, I was asked to leave the ward where I was on placement, reported to the head matron and eventually my placement was suspended. After a review of the incident, my Practice Assessor refused to continue to act as my assessor and sign off any of my practice competencies. I have since reflected on the whole incident and recognise that working with others would likely bring challenges. I am, therefore, more resilient and aware of my own emotions, anger and perceived feelings of humiliations in dealing with cultural differences, disagreements, misunderstandings and challenges that may arise out of poor communications and miscommunications at work. 3. Now What? – (Evaluating the Experience) · I need to do in order improve my interprofessional communication? · Other issues need to be considered if this communication is to be successful? · strategies or tools you can use to improve your interprofessional communication · will the application of this learning in future placements impact on how will you approach similar situations differently? The experience I have learnt from reflecting on the whole incident is that I need to uphold and maintain professionalism, patience and emotional resilience in dealing with issues at work or placements. The NMC CODE (2018) recommends that nurses and student nurses practice effectively. Practising effectively means that I would be a lot more assertive in my future decision making. I should have refused to go into the nursing office when I was told I could go in by the gentleman that was in the office – as there was a sign saying, ‘do not use this room’. The incident that happened, whilst at placement, which led to the suspension of my first placement has been a big learning experience for me as I have reflected on many things that could have been done better and differently. I will also not try and resolve issues I face at placement on my own again. Instead, I will always be mindful of my professional values and behaviour, as a nursing student and therefore report any issues to my Practice Assessor first, and then my Academic Assessor for support and management of any incidents. I recognise that I will be working with different people of many diverse backgrounds. I will treat each case of disagreement with the right response that it deserves. I will treat people and work colleagues as individuals and not their racial backgrounds. In the future, I will be very mindful of unfortunate utterances when upset, as these utterances can be interpreted by others present as something else even though my intentions might be the different. I will pay particular attention and considerations to the feelings and perceptions of those around me. I will also, always, be mindful of how my behaviour may affect and influence my colleagues and patients around me at work. I will also always practice mindfulness, especially, in key moments when I feel upset or angry about any incidents at placement or work.
