Original Question

Topic: A.B. is a 15-year-old, who was assigned female at birth and lives at home with her parents and cousin. She reports to her primary provider expressing that she “feels different” in comparison to her female friends and classmates. Since A.B. was about five or six years old, she explained that she has always loved doing things that girls did not want to do. Even when playing with female friends, she would pretend to be masculine characters or male characters. She wanted to play sports, did not want to wear “girly clothes,” and she has always had trouble making friends with other girls her age but has always had a male best friend. She expresses that she has a crush on a girl but has visions of herself being that girl’s boyfriend, not girlfriend. As she continues to get older, she dresses in clothes from the male department, cut her hair shorter and even has thought of a new name. She wants nothing more but to be seen as a male. She has told her parents that she wants to change her name, and has even researched getting plastic surgery to remove her breasts. She expresses an internal conflict with being a girl, that being a girl feels wrong and she would like to move forward with becoming a male in entirety. She expresses struggling with this internal conflict and afraid of how her family will support a permanent change. A.B. has no past medical history. She is in the 10th grade and struggles socially at school but is a great student academically. He is an only child, but her cousin was adopted by her parents and she has a great relationship with her. Her parents are both attorneys, her mother works at a real estate firm and endorses a past medical history of major depressive disorder and anxiety. Her father works in a private practice, reports hypertension and works extremely long hours. He endorses working extra hard to provide for his family due to growing up watching his single mother struggle. Based on A.B.’s symptoms, she is suffering from gender dysmorphia disorder (GD). Gender dysphoria (GD), according to the Diagnostic and Statistical Manual of Mental Disorders (DSM 5), is defined as a marked incongruence between their experienced or expressed gender and the one they were assigned at birth (Garg, G., 2023). GD was previously identified as gender identity disorder. A.B. has expressed at least six symptoms that are highlighted with GD and with specifically the one that must be present which is a strong desire to be of the other gender. She dresses like a male, has cut her hair shorter, expresses interest in the same sex, wants to permanently change her appearance, has trouble making girl friends, prefers male dominant sports, and insists on making friends with more males. Based on the case, select management strategies you would recommend. Provide support from a scholarly source to support your decision. Management strategies for GD would be psychotherapy to help manage feelings, incorporate boundaries and assist with commuication skills for guiding conversations with A.B.’s parents, relatives and friends. Group/Supportive therapy should also be considered for A.B. as there are positive outcomes related to patients such as development of social skills, growing confidence, relating to others and learning new coping strategies. Individual, family, and group therapy are important for children to explore and counsel on gender preference (Garg, G., 2023). Describe a minimum of two referrals you would make for the client and include your rationale. Referral to a mental health provider will be important for A.B. This will be important to screen for other disorders that are seen with GD such as anxiety and depression. Another referral should be to an endocrinologist if she is interested in hormonal therapy. Due to the want to be viewed as a male, a referral for an endocrinologist for hormonal therapy may also be appropriate. Necessary referrals should be made according to the patient to give them a more robust support system; the added anticipation of puberty is of concern, so hormonal treatment and psychotherapy should be considered simultaneously (Garg, G., 2023). Analyze one ethical or legal consideration related to the treatment of a client with your assigned diagnosis. Informed consent related to clients with GD should be a priority for providers. It is important that providers inform not only the client but their parents with sufficient and accurate information related to all treatment options, including all risks not just benefits. It is also important to discuss the possibility of regretting decisions and irreversible outcomes such as plastic surgery. Evaluate the potential role of the family in the treatment of a client with the assigned diagnosis. What additional support may be necessary if the client’s family is unwilling or unable to participate in treatment? Parents play a crucial role in the care and treatment of a child with GD. Ensuring a supportive and safe space for the client is important, advocating for the fair treatment and rights of the child, and ensuring that respect for them starts at home. If a client’s family is unwilling or unable to participate in treatment, first encourage professional help. This can provide guidance for the parents as a means to better understand the situation that their child is going through. Recommend organizations such as PFLAG and The Trevor Project for more information regarding GD including books and support groups. Knowlege can be important to facilitate a better understanding and can promote support. If the parents are absolutely unwilling, it can be due to grief or loss of the gender of the child they gave birth to. Be patient and open to further communication. Respect and kindness goes a long way. Encourage family therapy to assist with open and honest communication between the parents as well as the family as a whole.
