Scenario – You are a nurse working in the Accident and Emergency department who has been asked to meet with Mohi to assess his mental health and risks to self and others. Mohi is currently living in Sydney, Australia. He moved to Australia from Tonga three years ago on a student visa with a scholarship for his studies. He was studying engineering at a Sydney University but found the course difffficult so left his studies twelve months ago. He has been working casually in warehouses and doing some gardening work since then. He hasn’t applied for any other Australian visa. His family had big hopes for this study and future employment as an engineer. His parents, two younger brothers and one younger sister continue to live in Tonga. They are struggling to sustain a regular family income. They can only afford electricity intermittently and are visiting a food bank for some meals. Mohi hasn’t felt able to tell his family that he didn’t finish his course. Rather than being able to provide them with a future of increased income, he has now built up a significant debt from his studies. He tries to send them as much money as he can from his casual work. He presents to Accident and Emergency with the police and ambulance. He had called a telephone helpline expressing suicidality. The helpline were concerned about his immediate safety so called the police and ambulance. He disclosed to the emergency services, that he had taken an overdose of over the counter medication. He also disclosed he had been experiencing low mood, insomnia, early morning wakening, weight loss and increasing guilt about his family’s situation for the last six months. At the time of taking the overdose, he wanted to die. The police and ambulance scheduled him under the Mental Health Act to transfer him to hospital to maintain his safety and further assessment. He had been reluctant to go to the hospital again. He had been admitted to a mental health ward soon after arriving to Australia when he was experiencing distress at adjusting to the cultural change and the start of his studies. He felt that the hospital over medicated him with powerful drugs rather than offering a helpful place to talk and practical support.
