Transcription Policy & Procedure for History and Physical Completion Purpose: Ensure the completion and accessibility of the history and physical (HP) in the patient record within 6 hours of dictation. Rationale: Patient safety and continuity of care. Policy: All dictated history and physical reports have a high priority and require turnaround from dictation to transcription within 6 hours of dictation. Exception: History and physical reports dictated after 11:00 pm and before 6:00 am each day. Procedure: Emergency HP dictation is assigned the report type of 01 by the provider.
