PICOT Statement: In patients experiencing postpartum psychosis, how do long acting injectables (LAIs) compare to oral antipsychotics regarding readmission rates to acute psychiatric facilities? When working with patients going through psychosis, an event in which they are not in their right state of mind, there can be many ethical issues that come about, especially concerning treatment. Postpartum psychosis is a rare, yet extremely severe condition that can affect a mother at any point during her postpartum experience. According to Postpartum Support International, postpartum psychosis is when a mother experiences symptoms such as delusions, hallucinations, hyperactivity, and paranoia just to name a few. (Postpartum Support International, n.d.) These thoughts can often become violent or dangerous for the mother and baby and therefore is considered an emergency. Because of these symptoms, many women in this condition are not able to make decisions or have a say in their plan of care, nor for their babies. One ethical issue that is often addressed when dealing with a postpartum patient in a psychosis event is beneficence, the idea of promoting one’s wellbeing. Ensuring beneficence when working with a mother experiencing psychosis should always be a priority for the healthcare provider when determining their treatment and plan of care. However, because the mother is unable to make her own decisions when in this state, beneficence can often be questioned when ensuring the mother is getting the best treatment for individual case. In the Journal of Psychopharmacology, the need for pharmacological intervention quickly is a necessity in patients in this psychotic state.
