Part 1 (EMS to ED) EMS are en route to local ED with an electrical line worker injured while restoring power to rural power grid after storm damage. Bystanders reported he made contact with high-voltage lines, fell 15′ from his bucket truck to the ground, and experienced a brief loss of consciousness. Upon EMS arrival to scene 12 minutes after injury, patient was A&O x4, though anxious & complaining of 9/10 pain to neck, back, and arms. EMS applied C-collar and non-rebreather (NRB) mask delivering 15 Lpm of Oz. EMS noted obvious circumferential burns to entirety of both arms; arms are charred & blackened, axilla are red & blistered, and dry, pale white-ish areas are noted to palms. EMS were unable to establish PIVs but did place an intraosseous (10) catheter in proximal tibia and infused a 500 ml bolus of normal saline (0.9% NS). After 8 minutes of EMS time on-scene, pt is scheduled to arrive in local ED in 10 minutes, ~30 minutes after time of injury. VS on scene were: HR = 130; BP = 150/73; RR = 24, shallow & rapid; Sp02 = 90% on RA (this improved to 95% on 15 Lpm of O2 via NRB); EtCO2 = 35 mmHg; initial pain was 9/10 to neck, back & arms; after 4mg Morphine given x1, pain was reassessed as 6/10. 1) How should ED nurse prepare for patient arrival?
