Write a follow up Psychiatric visit HPI, social, MSE, screening tools, diagnosis and recommended Plan for an 89years old female who currently resides at sharon care, a skilled nursing facility. I am a new provider, taking over patient’s care from a previous provider. She has diagnosis of Anxiety, depression and auditory hallucinations. She is currently on Quetiapine 25mg at HS for auditory hallucinations, Melatonin 3mg PRN for Insomnia and Doxepin 10mg HCL for depression. Patient said her mood is up and down and she sometimes still gets auditory hallucinations. She is well groomed and HOH. She said I am ok as expected but I am loosing my mind. I cannot do anything till I walk on my foot. She is working with PhYSICAL therapy for strengthening. Appetite- I am rarely hungy.
