She has a history of hypertension and Diabetes Mellitus type 2 that is managed with Metformin. Upon admission to the ED she had a troponin T 0.04 mcg/L. Her ECG shows no changes except tachycardia. She reports that the began having chest pain 2 days ago. The first incident occurred when she was walking upstairs, the other times she was doing house chores and laundry. The chest pain worsened and she became short of breath and had indigestion. Each time she sat down and waited for the pain to stop, then she resumed her normal activity. Her current vital signs include: BP 168/94, temp 38.5C, HR: 110, O2 sat 95%, and RR: 22. Her blood glucose level is 134. The orders include: O2 via nasal canula 2L/min nitroglycerin tablet 0.3 sublingually every 5 minutes, repeat up to two times within 15 minutes if pain persists. Call MD if pain persist past three treatments. bedrest and observation for the next 4 hours, if pain resolves patient can be discharged with medications, teaching, and referrals. If pain is unresolved patient may need to remain in the hospital for observation for up to 24 hours.
