Original Question

Physical well – being and Health Magda health was good until about 5 months ago when she fell and broke her hip. She is experiencing chronic pain after receiving surgery and rehabilitation (Walden. University, 2022). Assessment of Caregivers After breaking her hip Magda now requires additional assistance. Magda was receiving care from a woman who attended Magda’s church. The church member would check in on Magda a couple of days a week. Magda liked having the lady come and assist her. Helen would have liked for the lady to come more often but could not afford it (Walden University, 2022). Magda’s son and daughter – in – Law were using money from their vacation and retirement savings to pay the lady from the church. Magda also received assistance from her daughter – in – law Helen who went to Magda’s house on the days the church member was not there, sometimes twice a day (Walden University, 2022). In order to save on cost Helen made the decision to have her son Alec stay with Magda since he had a good relationship with his grandmother, and it would take some of the burden off her (Walden University, 2022). Magda was not included in this decision and was unhappy she liked to have the women from church check in on her. This decision ended up causing more stress to Helen because Alec was not caring for his grandmother. He was hardly there and was stealing from his grandmother to support his drug addiction (Walden, University, 2022). Once Helen discovered what was going on she decided not to tell the police, Magda or her husband. She is now coming over twice a day to bring supplies and medicine and to check on Alec and Magda (Walden University, 2022). This situation could cause Helen to experience more stress and burn out. Physical environment Magda is no longer in a safe environment. She is living with her grandson who is a recovering drug addict and is stealing from her and taking her medication. Social functioning Prior to her hip injury Magda was able to attend weekly family dinners, she was able to drive which allowed her to shop and go to the Greek Orthodox church. She had more opportunities to socialize with other. Since her surgery she does not get out much and does not get to interact with others like she did before. This causes Magda to feel down and lonely. Ability to perform ADLS Prior to breaking her hip Magda was very self-sufficient and able to care for herself. After her surgery she now requires some assistance. She is not driving so she would need someone to go with her to do her shopping and may need some assistance with house cleaning. Cognitive Capacity Despite Magdas recent diagnosis of dementia, she is aware of what is going on. She is able to tell her daughter – in – law that her grandson is never around to help her and is able to tell Helen that her medications need to be refilled. However, as time goes on, she will experience cognitive deterioration due to her dementia. Psychological well – being and health Magda was recently diagnosed with early signs of dementia. She may be experiencing signs of depression since she is not able to get out on her own and socialize with others outside of the home. Magda stated she feels that she is a burden to her family. She feels no one wants to care for her. Her family is not including her in the decisions about her care. Describe ways you would have supported Magda’s self-determination through the assessment and treatment-planning process. Include questions you would have asked Magda and her professional support system (doctors, nurses, etc.) to gain further insight into the situation. I would find out resources were available to help with paying for the care she needed. I would talk with Magda, her son and daughter in law to find out what services they felt were needed. I would make sure Magda was included in making decisions about her care. I would contact Generations Mental Health to find out what services they had available. I would provide the family with information about programs were an attendant could come in and assist Magda with cleaning her home, taking her to appointments, shopping, church and social event. I also would provide them with information on live in facilities that could provide care she many needs in the future, due to her recent diagnosis of dementia. I would provide Helen and her husband with information for service available for care givers as well as support groups they would benefit from. I would contact her doctor’s office to find out what services they felt she could benefit from. Reference Walden University, LLC. (2022). Social work case studiesLinks to an external site.
