She has been diagnosed with severe depression, and needs encouragement to eat, drink, and to attend to her personal care. She has lost nearly 8kg since she first became unwell (about 6 weeks ago). Maya works as a manager at a local museum, a job she loves, but she has been on furlough for 3 months. She is divorced and lives alone in a house she owns. She enjoys cooking, reading and walking. She has 2 grown daughters who live with their partners; she sees them every few weeks. This is her third episode of severe depression. She was admitted after telling one of her daughters that she wished she were dead and disclosing that she had been stockpiling medication to use for an overdose. On admission she was mute, crying, was refusing food and fluids, and did not want to get out of bed or shower. She did not want to attend any activities or occupational therapy groups. On the ward, she was placed on one-to one observations as she was deemed a potential suicide risk. However, after ten days she appears to be responding to medication (40 mg fluoxetine daily) as today she managed to get out of bed unprompted, and yesterday she showed some interest in a programme on television and engaged in some conversation with her daughter who came to visit. Today, you have half an hour to spend with Maya. When you go to sit down with her, you notice that her skin appears dry and she is tearful. She wants to go home and will be missing her elder daughter’s 30th birthday party which is planned in ten days’ time, and for which she was going to be baking a cake. She says she feels as if she is a “hopeless” mother and “good for nothing”. She is also concerned about some bills she said she needs to pay and some library books that are overdue. She is complaining of nausea and heartburn, known side effects of fluoxetine.
