Case for the assisnment study Pravin Kumar is a 40 year old gentleman who lives with chronic back pain following a car accident and has depression. Pravin has accessed talking therapies with good effect over the years and currently takes anti-depressant. Pravin has an elevated BMI of 29 and type 2 diabetes. Pravin has not attended regular appointments for his diabetes and is finding it difficult to mobilise due to pain and is becoming increasingly isolated. Presenting complaint- back pain and small ulcer on left foot under 3rd metatarsal head. PMH- Depression diagnosed 6 years ago, long term back pain following car accident, Type 2 diabetes diagnosed 2 years ago and hypertension. Medications- Fluoxetine 20mg OD Naproxen 500mg BD Metformin 500mg TDS Tramadol 100mg BD Lisinopril 2.5 mg OD Family History – father diabetes and heart disease Social history – Pravin lives alone in a rented flat.
