Hello Everyone I currently work in an express/urgent care clinic and I love working there. However, something that concerns me is the paper chart system we still use. When a patient checks in, we have them put their name, date of birth, and reason for the visit on a piece of paper and attach it to an encounter form that has their address, number, insurance, and other information that the doctor will need. Next, we put both their forms in a one-pocket folder, turn the papers around so none of their information is displayed, and put the folder outside their room and wait for the providers to see the patient. This workflow could lead to a HIPAA violation. If someone walks by the room they could easily flip the papers around and have access to the patient’s personal information. The staff does are best to monitor the patients but sometimes we have emergencies and are pulled away to other situations within the clinic. I recently brought up my concerns at a staff meeting. Ways we could improve the workflow by getting rid of the paper charting system and doing everything through the computer system, this way we are protecting a patient’s personal information. Evidence-based practices implement patient values, expert opinion, and clinical experiences to diagnose, reduce risks and treat patients (Houser, 2018).
