Nurs 4021 Case Situation: As a community nurse working in a small-town elementary school, you see a significant increase in asthma-related absences, and in children who are coming to your office with shortness of breath during the school day. Although each child’s situation is unique, they seem to share a number of characteristics. Most of them belong to low-income families in which both parents work. They all live in the neighborhood surrounding the school. There is considerable construction in the neighborhood that keeps everything covered in dust. In addition, the schoolyard is a home for the production of pollen by weeds. The school is old and previous water leaks have led to widespread molding and mold in some of the classrooms and in the heating vents. Most children use albuterol inhalers when they have difficulty breathing, but few of them use nasal steroids regularly or their albuterol before schoolyard activities. Many of the children say their doctors have prescribed nasal steroids, but their families can’t afford the medications as prescribed. When talking to some of the primary care providers in the community, you find that they are frustrated by their inability to control asthma in many of the children and that their offices are filled with children with asthma attacks, both mild and severe. What would be the first actions of the community health nurse in this situation? How would you include community members in making the care plan? What should members be involved in?
