Original Question

The management of Graves’ disease is aimed at reversing or controlling the excessive thyroid hormone production and mitigating the autoimmune stimulation of the thyroid gland. Each treatment targets different points in the pathophysiologic cascade creating varying outcomes and long-term control of the disease. In our patient’s case, beta-blockers provide rapid symptomatic relief by blocking β-adrenergic receptors, decreasing the body’s sympathetic responses. Propranolol is used in this case study to reduce symptoms of tachycardia, tremor, palpitations, and anxiety which are due to heightened sympathetic nervous system activity. Thionamides, such as Propylthiouracil (PTU), act directly at the thyroid gland to inhibit thyroid peroxidase, thereby blocking the synthesis of T3 and T4. This medication helps to restore euthyroidism by reducing hormone overproduction but it does not address the underlying autoimmune cause. Radioactive Iodine Therapy (RAI) is a definitive treatment that utilizes the thyroid gland’s natural affinity for iodine. The radioactive isotope is absorbed and causes localized destruction of thyroid tissue, reducing the gland’s capacity to produce hormones. Patients may continue to have extra-thyroidal manifestations, such as ophthalmopathy, which can worsen post-RAI. Surgical treatment options include removal of the thyroid gland. This provides immediate resolution of hormone overproduction by removing the entire gland. Like RAI, it does not impact the autoimmu
