Original Question
Jamie loves it when the canned goods are on sale in her favorite grocery store. She usually buys the cans in bulk and store them in her pantry and not worry about buying it often. It helps when she has the item when she needs it. Like that one weekend, when the family decided to go on an outing. Finding some left-over tortilla and cheese from the day before, she thought about making burritos. Although happy to find canned tomatoes and beans in her pantry, Jamie was a bit surprised; she did not remember when she bought them. She noticed that one of the cans was bulging, but thought that her 5-year-old daughter, Amy, had played with them and dropped them on the floor before she stored them. Following the quick recipe, her burrito was ready in 15 minutes. The family had fun at the beach and Amy devoured the burrito after her swim. Jamie thought it was a bit too salty, but happy to know Amy liked it. “Too bad Ken did not like beans,” she thought about her husband, who ate a sandwich instead. However, the following day, Amy fell ill with stomach cramps. She vomited several times. Soon after, she complained about having dizziness and blurry vision. Jamie was concerned because her daughter just had a visit to the eye doctor, and had excellent vision. When Amy started having slurry speech, she and her husband rushed her to the ER. By the time they reached the hospital Amy had shortness of breath. Strangely, Jamie began to feel nauseated and dizzy as well. Her legs felt weak and she could no longer stand up. She thought her cold that started a few days ago just got worse. When the doctor found out about what the family ate over the weekend, he quickly ordered that Jamie and her daughter were placed in a mechanical ventilation and an antitoxin to be administered. When released from the hospital 10 days later, Jamie was glad that the doctor made the right decision for her and her daughter; otherwise paralysis and death would be the consequence. Answer thoroughly the following questions to explain to the patient: 1. Etiology: What is the causative agent for this disease? What are the deciding clues from the case that help you find out the organism? What is the portal of entry? How does the organism cause the signs and symptoms mentioned, including paralysis? 2. Other possible causes: List two other microbes that cause symptoms similar to some of the symptoms in the case. What information helps you eliminate them? 3. Details of the microbe: What are the identifying characteristics of the pathogen that causes the disease? What tests would be done to confirm the etiology and what results would be expected from the tests? 4. What is the standard treatment for the disease? (Be specific, a general term such as antiviral or antibiotic doesn’t inform much). Why were the patients placed in mechanical ventilation? What is the reason for the use of an antitoxin? What is the likelihood of recovery from the illness?
