Explain the ethical dilemma and any challenges this dilemma presents Ethical Dilemma Leslie Evans, a 32-year-old woman with cystic fibrosis, wants to live no matter the cost. She has been married for less than five years and has a devoted husband and caring family. Leslie is ventilator-dependent and has been transferred to ICU after developing pneumonia. Although she was insistent that she was on the heart-lung transplant list of two centers, both institutions declared that she no longer fit the criteria, yet she is unaware. Leslie is determined to survive. Every possible therapy and intervention has been tried, and her ventilator is set at the maximum setting, but she still becomes increasingly hypoxic. Any slight sound startles her, and she awakens terrified, hypoxic, and diaphoretic. Attempts to suction her result in severe hypoxia, and her heart rate drops into the 40s. When she is quiet, in a darkened room with her husband holding her hand, she gets rest. This is not what Leslie wants. She wants to get better and live. She has asked to be turned, suctioned, and percussed despite her hypoxia. The nurse does what the patient and family ask yet feels torn that the patient is unaware that she is no longer on the transplant list.
