Emergency Department Note: Chief Complaint: Injury to right arm due to playground accident. HPI: This is a 16-year-old boy who fell from the playground equipment at a public park. The patient presented to the ER with an injury to his right arm indicating a possible fracture. He suffered no other injuries as a result of this accident. He suffered no loss of consciousness or head trauma. The patient is also a Type 2 diabetic and is on long-term Humulin Insulin. PE: HEENT: Normal. Neck: No adenopathy, supple. Lungs: Clear. Abdomen: Soft. Cardio: Normal sinus rhythm. Extremities reveal obvious redness and swelling of the right upper arm. The humeral bone appears to be somewhat deformed. Radiological exam taken at the hospital reveal a displaced transverse fracture of the mid-humeral shaft. No bleeding or hematomas noted. Vascular exam: Bilateral venous pulses in the posterior tibial and dorsalis pedis. Capillary refill time is normal. Neurologic exam: Normal. Motor and sensory exam: Normal, with no paresthesia or numbness to the right arm. PLAN: Patient will be taken immediately to the operating room where I will surgically correct the fracture. Operative Note: Preoperative Diagnosis: Fracture of the right arm Postoperative Diagnosis: Fracture of the right arm ER Procedural Note: The patient was brought to the operating room. A closed reduction of the fracture was performed without complications. A long-arm cast was placed on the patient for stabilization of the repair. Extensive padding was placed over the fracture site and the patient was brought to the recovery room. The patient tolerated the procedure well. Prognosis is good. The patient is to return for follow-up with me in one week. He is to continue his insulin as he had been. The patient was informed to watch for any complications and to contact us immediately if there were any questions or concerns. ER Visit was: History=Problem-Focused; Exam=Problem-Focused; Med.Dec.
