Case #1: Mr. J.R., 52 Year old male with chest pain HPI: A 52-year-old male presents to the office with chest pain that started about 12 hrs. prior to arrive. He describes it as a dull pain in the center of his chest, pain is present 4/10. He denies nausea, palpitations, radiation, dizziness, or dyspnea. He took aspirin before bed but the pain was still there when he woke. Medical history: GERD and low back pain. Medications: Famotidine for GERD, Ibuprofen as needed for the back pain Allergies: NKDA Social history: Reports he drinks 3-4 alcoholic drinks per night and more on the weekends. He denies tobacco use or illegal drug use. He has no allergies. Family history: father deceased age 62 MI. Mother deceased age 90, natural causes, one sister with type 2 Diabetes. ROS: all reported as negative except for chest pain as in the HPI PE: Patient appears well developed and well-nourished. BP 138/88, P 90, RR 16. Temperature 97.6. General physical exam reveals no abnormalities. (consider all areas negative) ECG performed in the office reveals sinus rhythm with non-specific T-wave changes in leads II, III, and aVF.
