Describe an adverse clinical event that occurred in your unit. What were some types of ethical dilemmas that can arise from or during this event? When I worked on a medical/surgical/telemetry unit, we had a nurse who started a potassium chloride drip at a rate of 100 milliliters per hour or 40 milliequivalent (mEq) per hour. On this unit, our protocol was to infuse 10 mEq per hour or a maximum of 20 mEq per hour if the patient is on telemetry. I was the charge nurse that day and entered that room to help the patient with something unrelated. I noticed the potassium chloride at that rate and quickly changed it. I noticed the bag was almost full and the patient had not received much of it. I spoke with the nurse who immediately realized what she had done. She was very at that time and had made a mistake. We had to document the incident, but fortunately, nothing happened to the patient or the nurse. We attended a meeting to explain the circumstances to our leadership and the risk management department became involved.
