My clinical practicum setting is a primary care office, the population I typically see are older adults. The US Preventative Services Task Force has two preventative care screenings that are excellent for the older adults in my clinical practicum. The first one is Colorectal cancer screening and the second being Hypertension in adults screening. I have noticed that there are many patients that have hypertension and are not compliant with their medications or are not monitoring their blood pressure at home. I also see patients refusing screening colonoscopies because they feel embarrassed, and some do not like the prep prior to the colonoscopy. The preventative care provided at my clinical practicum is geared toward the older population. My provider uses the Medicare guidelines to determine which screenings are required and recommended for the older adults. The quality of preventative care I have observed is great. Every patient sixty-five and older that comes to the office for their annual wellness visit gets screened for cognitive impairment and depression. The office uses the Patient Health Questionnaire 9 (PHQ-9) to screen for depression and Six-item Cognitive Impairment Test (6CIT) to screen for cognitive function. I have not observed any disparities or bias in the care provided to different members of the population. Screening recommendations are presented to clients by providing education and giving them pamphlets, showing them educational videos, and reviewing posters in the examination room. Some patients are visual learners who the videos and posters are the best way to educate them. My clinical practicum site is lucky to have a multilingual staff. So, there is always someone that speaks a language other than English in the office. I have noticed the office does not have a language interpreter phone or video system like they have at the hospital I work for. There have been a couple of patients that have no insurance come into the office. Unfortunately, they were not able to get screened for any preventative services. Patients without insurance do not have enough money to pay for preventative screening and by the time they come into the office they already have the disease. For example, a patient with no insurance comes into the office already diagnosed with hypertension from 2 years ago. He stated he stopped taking the medication because he did not have money for refills or the doctor’s office visit. The blood pressure in the office that day was 190s/110s. I educated the patient on the importance of taking his medication and preventing him from having a stroke. I also gave him resources on where he can get medication at an extremely comfortable price. I would recommend the practice and community has free healthcare screening fairs at least once or twice a year for the vulnerable populations. The fair would give out resources and educational pamphlets to patients that have limited access to healthcare. There should also be an insurance representative that will guide individuals with no insurance on how to apply for free healthcare. Some individuals due to the lack of guidance or inability to read are not able to fill out an application for health insurance. I would also recommend to my clinical practicum site to have resources and or pamphlets on how to apply for free or low-cost healthcare. I would also recommend that my clinical practicum site have a language interpreter device because there may be a time when an office staff is off and a patient that needs an interpreter walks in.
