Original Question
Case Presentation Amanda Soto, a 22-year-old female, presents to an outpatient clinic with a chief complaint of headaches and problems sleeping. Yvonne Sanchez, the nurse at the clinic, remembers seeing Amanda in the clinic not long ago. She reviews Amanda’s medical record before seeing her and notes that Amanda has been in the clinic four times in the past 5 months, each time with vague symptoms, and each time she was sent home with a nonspecific diagnosis. The nurse reviews her social history and sees that Amanda is married and has two children, ages 2 and 4 years, and works as a receptionist. Amanda has no medical conditions and takes no medications other than oral contraceptives. When Yvonne enters the examination room, she observes Amanda sitting on the examination table with her knees held tightly against her torso and rocking back and forth. Yvonne asks Amanda to describe the symptoms she has been experiencing. Amanda states her headaches have been occurring off and on for a while, and she describes them as “intense” and often keeping her awake at night. As she talks, Yvonne notes a flat affect in Amanda’s communication pattern. She also notices bruising to her neck, just above the clavicle, and on her upper arms. When Yvonne asks Amanda the last time she experienced the headache, she tells Yvonne she cannot remember exactly, but perhaps 1 or 2 weeks ago. Yvonne suspects Amanda may be experiencing violence. She pulls out the Partner Violence Screening tool, available at her office, and asks Amanda the following three questions: 1. Have you been hit, kicked, punched, or otherwise hurt by someone within the past year? If so, by whom? 2. Do you feel safe in your current relationship? 3. Is there a partner from a previous relationship who is making you feel unsafe now? Amanda begins to cry and acknowledges she is in trouble. She states her husband has a drinking problem, and every couple of weeks, he gets drunk and becomes violent. He always feels badly after he becomes sober, but she has become frightened at what he will do to her or the children. She states that he has never hurt the children, but they see him regularly hit her. Amanda tells Yvonne she has contemplated leaving, but she does not think she can. She explains that her husband controls all of the money, and she has nowhere to go. She is sure she does not earn enough money to support herself and her children, and she is worried that he will track her down anyway. Amanda feels completely trapped and unable to remove herself from the situation. Case Analysis Questions: 1. What type of IPV does this case exemplify? 2. What were the clues that led Yvonne to suspect IPV? 3. What should Yvonne do as a next step?
