Original Question
Assign the appropriate codes for the following procedure(s). Include any modifiers, when applicable. MRI of the brain with the following interpretation by Dr. Johnson, performed at the local hospital: Multiple sagittal images were obtained with a TR 600 and TE of 20. Multiple axial images then were obtained with a TR 2000 and TE of 30/80. In addition, coronary images with TR of 2000 and TE of 30/80 were obtained. Findings: Sagittal images reveal effacement of sulci and heterogeneous low density in the right frontotemporal region. The cerebellar tonsils are positioned normally. The pituitary gland is normal in size. Axial and coronal proton spin density and T2-weighted images reveal an area of generally increased density with the posterofrontal right temporal basal ganglial region. The outer margins of this high-signal finding are consistent with edema. In the center is a large mass, most likely representing glioblastoma. There is a shift of the midline structures that include the corpus callosum and ventricles, with some compression of the right ventricular system. There is no evidence of spread across the corpus callosum by this mass. Impression: Right basal ganglial-posterofrontal-right temporal lobe mass with surrounding edema, most compatible with a glioblastoma. There is a shift of the midline structures, but no evidence of spread across the corpus callosum to the other hemisphere. Dr. Johnson’s code
